A patient is discharged from the hospital and home care will
help prevent re-admission.
New diagnosis requires in-home teaching to reinforce
physician's plan of care, avoiding complications or unplanned
hospitalizations.
Severity of illness at time of office visit requires
assessment & monitoring a home to shorten recovery time or
prevent hospitalization.
The patients medical condition is compromised by physical
frailty, cognitive decline and/or social isolation and is best
resolved by teaching family or designated caregiver's in proper
disease management and treatment techniques. The patient has an
exacerbation or worsening of a medical condition requiring new
medications or treatment.
Why is Quality Home Health "Just like family"?
Our Agency is owned and operated by Tennesseans and has been
treating patients and helping families in Tennessee for twenty
(20) years. Each patient admitted to our agency is assigned to a
care team to create continuity of care and improved
communication.
Quality Home Health Agency is an independent agency, not a
hospital based agency, therefore we must earn the trust of
patients and their physicians by providing excellent clinical
care and courteous service.
Our only business is home health, therefore we are able to
focus all of our efforts on the home care needs of our patient
and their family. The result is excellent care and highly
satisfied patients and physicians. We participate in both
patient and physician satisfaction surveys and they are
available for review at your request.
Our agency utilities and follows a Clinical Pathways for
over fifty (50) home care diagnoses. Clinical Pathways provide a
guideline for consistent, highly standardized care which enables
our staff to achieve optimal clinical outcomes in an efficient
manner.
