A patient is discharged from the hospital and home care will
help prevent re-admission.
New diagnosis requires in-home teaching to reinforce physician's
plan of care, avoiding complications or unplanned hospitalizations.
Severity of illness at time of office visit requires assessment
& monitoring a home to shorten recovery time or prevent hospitalization.
The patients medical condition is compromised by physical frailty,
cognitive decline and/or social isolation and is best resolved by
teaching family or designated caregiver's in proper disease management
and treatment techniques. The patient has an exacerbation or worsening
of a medical condition requiring new medications or treatment.

Our agency utilities and follows a Clinical Pathways for over
fifty (50) home care diagnoses. Clinical Pathways provide a guideline
for consistent, highly standardized care which enables our staff
to achieve optimal clinical outcomes in an efficient manner.
Who is Eligible for Home Health?
Medicare pays for home health services if all four of the follow
conditions are met:
Patient is homebound.
Patient needs the skilled services of a Registered Nurse, Physical,
Speech or Occupational Therapist.
Patient is under the care of a physician who establishes the
home health plan of care.
Services are provided by a Medicare certified agency.
What are your Hours of Operation?
Office Hours 8am-5pm Monday-Friday. Professional staff available
24 hours / 7 days per week in all locations.
Do You Accept My Insurance?
We accept Medicare, most group insurances, private pay & limited
TennCare plans. Call any office for specific answers regarding your
plan.
Will the patient's care be covered by Medicare?
