Prevention of Readmission
A patient has been discharged from the hospital and home care will help prevent readmission or aid in continued recovery. It is always our goal to help patients get better and regain independence.
  In-Home Teaching
A patient has an exacerbation of an existing chronic problem, a new illness or medicine and needs to learn to care for him or herself. Or, the condition is best resolved by teaching family members proper management and treatment techniques.
Physical Therapy
Providing services that help improve strength, mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries/disease.
  Speech Therapy
Helping individuals improve the quality of their life by maximizing their communication ability (Listening, Reading, Speaking, etc.).
     
Skilled Nursing
Provide complete and specialized home care which requires specialized skills and training.
  Home Health Aide
Provides assistance with personal care (bathing, dressing, oral hygiene, and light housekeeping).
Occupational Therapy
Teaches patients to manage their environment by focusing on fine muscle training and assists in rehabilitation efforts. The goal is to improve the patients’ quality of life by helping them compensate for limitations.
  Medical Social Services
Evaluates the social, emotional, and economic needs of the patient and provides assistance when necessary.
Wound Care Therapy
Provide the highest standard of wound assessment and management while accelerating recovery time and reducing pain.
  Assessment & Monitoring
The patient is closely watching for signs of complications or worsening. Because the patient is treated at home, he or she will receive the full, one-on-one attention of the clinician or caregiver. Our nurses and therapists serve as a liaison for any changes in the patient's condition as well as providing nursing or therapy interventions.
